Request for Second Review: Add New Supporting Documents
When submitting a Request for Second Review, DaisyBill automatically attaches the SBR-1 Form and the modified CMS1500 to the Second Review submission.

To choose additional supporting documents to attach to the Second Review, follow the steps below.
Step 1. From the Second Review, under ‘Supporting Documentation Attached?’, select ‘Yes’
Step 2. Click ‘Save’
Step 3. On the Bill Page, to add additional ‘Supporting Documents’, click ‘Add’
Step 4. Click ‘Submit’, then confirm and ‘Choose Second Review Documents’
The pop-up displays a list of all Supporting Documents attached to bill. New Supporting Documents are already selected. If desired, select additional documents to transmit with the SBR from Supporting Documents Previously Submitted.
Step 5. Review warnings, if any
Marking ‘No’ or leaving ‘Supporting Documents Attached’ blank on the Second Review Page (Step 1) will result in a yellow warning. To resolve, click ‘Cancel’, edit Second Review and select ‘Yes’ for ‘Supporting Documents Attached’.
Marking ‘Yes’ for ‘Supporting Documents Attached’ on the Second Review Page (Step 1) and failing to upload new Supporting Documents will result in a yellow warning. To resolve, click ‘Cancel’ and upload new Supporting Document(s).
Step 6. ‘Choose a Bill Submission Method’, and click ‘Submit’
