Labor Code Specifications

The CMS-1500 Form must:

  • Be the only form used by healthcare providers to file compliant workers’ compensation payment requests for services rendered, per uniform billing requirements in California Labor Code: 4603.4.
  • Have all information on the paper version of the form typewritten, according to the California DWC Medical Billing and Payment Guide.
  • Contain all of the required and situational information listed in the Medical Billing and Payment Guide. Appendix A lists the information which is required, situational, and optional when filling out the bill form.
  • Be filled out using ICD-10 Diagnosis Codes for all dates of service on or after October 1, 2015.
