Physician's Report

The Virginia Workers' Compensation Commission requires treating physicians to file an Attending Physician’s Report following a patient’s initial examination and upon request.

The Code of Virginia ( § 65.2-604) specifies that this report shall be completed and filed upon the request of the injured employee, employer, insurer, or a certified rehabilitation provider.

Report Form

Form 6 is required by the Virginia Workers’ Compensation Commission for a Physician’s Initial Report of Injury. The report must be made using or by attaching a report to the form.
