CMS-1500 Form Instructions
The state of Nevada has no specific requirements for any billing forms. It is recommended that providers compliantly complete the CMS-1500 Form.

CMS-1500 Required Billing Documentation
For a complete bill, in NAC 616C.149, Nevada requires the provider to submit the following supporting documentation with the CMS-1500 Form, when applicable.

Any bill for an office visit that is billed under codes 90000 to 99999, inclusive, must include a written report concerning the history of the injured employee, a comprehensive evaluation of the injured employee’s health condition or an evaluation of specific health problems of the injured employee, any decision made concerning the treatment required by the injured employee and all forms for submitting a claim to the insurer or billing reports that are requested by an insurer. Such a bill is not required to include a special report that is specifically requested by an insurer and is required to be billed under code 99080. |

Timely Filing
Medical Bills must be filed within 12 months of the date of service for services rendered in Nevada.
