Filing Requirements

The Texas Division of Workers’ Compensation requires any Treating Doctor, Doctor Selected by the Treating Doctor, or Insurance Carrier-Selected RME Doctor to file a Report of Medical Evaluation. The report  must be filed within 7 working days of the date of the evaluation—or receipt of all medical information necessary to certify the report—and must then be sent to the following parties:

  • Injured Employee (or their representative)
  • Insurance Carrier
  • Treating Doctor
  • Division of Workers’ Compensation

DWC Designated Doctors conducting a Medical Evaluation must file the report even if the patient has not attained a Maximum Medical Improvement (MMI) designation. The report must then be filed within the same timeframe of 7 working days and sent to the same relevant parties.

The Division of Workers’ Compensation requires the Texas Report of Medical Evaluation Form or the DWC-069 to be completed in the above cases.
