Patient Discharge Report

Arizona Revised Statute § 23-908(A) stipulates that treating physicians shall “from time to time a full and complete report of every known injury to the employee arising out of or in the course of employment and resulting in loss of life or injury” with the Industrial Commission of Arizona.

The Arizona Administrative Code R20-5-113 further specifies that upon discharging a patient, the physician shall determine whether the patient has sustained any functional impairments as a result of the injury and, if so, rate the percentage of impairment using the most recent AMA standards for the evaluation of permanent impairment.

The Arizona Administrative Code does not require any specific form for filing a Patient Discharge Report; however, R20-5-113 does require the physician to provide a final signed report detailing:

  1. The rating of impairment; and
  2. The clinical findings that support the rating.
