Physician Reporting Requirements

Florida requires providers to submit supporting documentation when submitting medical bills.

Providers must submit the DFS-F5-WC-25 form following an initial evaluation, subsequent evaluations, and for transferring care to another provider.

The table below lists the reports that accompany a bill, along with submission deadlines to fulfill supporting documentation requirements.
