CMS-1500 Form Instructions

Per Section 23.30.097 of the 2023 Alaska-Medical Fee Schedule, providers are required to submit an “appropriate form,” but no further information is provided. The state has indicated that most payers in the state accept the CMS-1500 Form but no specifications are provided.
