Alabama does not specifically require any supporting documents for a complete bill, though providers are subject to potential on-site audits. Chapter 3 of the Alabama DIR Administrative Code provides further information regarding on-site audits.

CMS-1500 Form Instructions

Per Regulation 480-5-5.22 (1) (a) 1, the state of Alabama requires providers to compliantly complete the CMS-1500 form, but provides no further information.
